Before the semester begins each Delight chapter needs to submit this informational contract to Delight HQ by January 5th. Only one form needs to be submitted from your chapter. After submitting this form we will send you your chapters books and all other needed material for the semester. Please fill this out in its entirety. Partial forms will not be accepted.
